Mentor Interest Form

The purpose of this form is to collect information from prospective mentors who are interested in participating in the Hertz Community Mentoring Program. The information you share will help us recommend strong mentor/mentee matches. Please note that your responses may be shared with potential mentees as part of the matching process so please do not provide confidential information.

Mentor/mentee matches are not made immediately. We collect information from interested mentors, and once we identify a mentee whose needs align with your expertise and interests, we will reach out to re-confirm your availability and interest before making an introduction.
