The purpose of this form is to collect information from prospective mentors who are interested in participating in the Hertz Community Mentoring Program. You join a group of fellows across the career spectrum who are committed to giving back via expertise and guidance. The information you share will help us recommend strong mentor/mentee matches.

Please note that your responses may be shared with potential mentees as part of the matching process so please do not provide confidential information.

Mentor/mentee matches are not made immediately. We collect information from interested mentors, and once we identify a mentee whose needs align with your expertise and interests, we will reach out to re-confirm your availability and interest before making an introduction.
