Mentee Intake Form
The purpose of this form is to collect information from Hertz Fellows interested in participating in our Hertz Community Mentoring programs:
Ongoing Mentoring
You’ll be matched with a mentor to help you navigate a particular career stage or decision. Recent PhD graduates are paired for two years to support the transition out of graduate school as part of our Early Career Mentoring program. Fellows at other career stages are paired for one year.
Entrepreneurship Mentoring
You’ll receive our Entrepreneurship Resource Guide and access to a list of Fellow entrepreneurs who offer candid, as-needed advice as you explore the startup path.
Based on your responses and career stage, our team will determine the best fit and follow up to make an introduction.
